Requests on Behalf of a Patient (Non-Patient Inquiries)
If you’re requesting information on behalf of a patient—such as medical records, bills, or account details—please contact our Patient Accounts team. For privacy and security reasons, we do not process third-party requests through online forms or email.

When you call, please have:
- Patient’s full name and date of birth
- Your relationship to the patient (e.g., attorney, insurer, employer, care partner)
- Dates of service and the type of information you’re requesting
- A valid authorization to release information (as required under HIPAA)
Important: We cannot discuss or release any patient information without proper authorization.
